Master's students

  • Major Professor Confirmation
    • This form must be submitted to the Graduate Program Administrator by the first day of Spring Break of the first year of the MA program.
  • Advisory Committee
  • Program of Study
    • The form must be submitted on the proper form with approval by the student’s major professor, the departmental Director of Graduate Studies, and the dean of the Graduate School. This step should be completed by Friday of the second full week of classes of the semester in which degree requirements are completed.

Thesis Track MA Students

Non-Thesis Track MA Students

Ph.D. students

  • Major Professor Confirmation
    • This form must be submitted to the Graduate Program Administrator by the first day of Fall Break of the second year of the Ph.D. program. (Accelerated Ph.D. students must submit by the first day of Spring Break of the first year of the program)
  • Advisory Committee
  • Program of Study Planning Form
  • Final Program of Study
    • Ph.D. students must submit a Program of Study Form for approval by the dean of Graduate School by the time the notification of the preliminary oral comprehensive examination is given. You must make sure that this form is approved before your oral comprehensive examination is scheduled.
  • Comprehensive Exam Announcement
    • This form must be filed by the Graduate Program Administrator. After completion of the written comprehensive examination, the oral examination is announced to the Graduate School and posted on the Graduate School website. The Graduate Program Administrator will file this form through GradStatus.
  • Application for Admission to Candidacy
    • This form must be signed by your Major Professor and the Director of Graduate Studies and filed with the Graduate School. You must initiate this from through GradStatus.
  • Dissertation Format Check
    • Before defending your doctoral dissertation, you must submit one complete copy of your thesis or dissertation for a format check with the Graduate School.
  • Electronic Thesis & Dissertation (ETD) Submission Approval [Instructions]
    • Before scheduling your doctoral dissertation defense, you must submit the final ETD Approval Form and a corrected copy of thesis/dissertation.
  • Dissertation Defense Announcement
    • This form must be filed by the Graduate Program Administrator at least two weeks before the date of your defense. The Graduate Program Administrator will file this from through GradStatus.
  • Dissertation and Final Examination Approval
    • This form must be ready at the time when you take your final oral examination, that is, your dissertation defense. On this form, your advisory committee must approve your thesis or dissertation for the defense and record the results of your defense. You need to initiate this from through GradStatus.
